The Health Care Navigator will assist with navigating all aspect of the healthcare system. This team is part of the Novel Interventions in Children's Healthcare (NICH) Interventionist program (Pediatric Complex Care) that provides intensive non-clinical home, community, and medical system-based support, guidance and assistance for youth with chronic and / or complex medical conditions (e.g., type 1 diabetes, sickle cell disease, failure to thrive, pre- / post-transplant, etc.).
