Schedule : Mon-Fri 9am-5pm

Salary :  $ / hour

We are seeking a compassionate and organized individual to join MMHC & Brigham and Women's Hospital's team as a Patient Navigator. As a Patient Navigator, you will play a crucial role in assisting patients through their healthcare journey, ensuring they receive the support and resources they need.

Responsibilities :

  • Guide patients through the healthcare system, providing assistance and support at every step of their journey.
  • Serve as a liaison between patients and healthcare providers, facilitating communication and ensuring patients understand their treatment options.
  • Coordinate appointments, referrals, and follow-up care to ensure seamless healthcare delivery.
  • Advocate for patients' needs and rights, ensuring they receive appropriate care and support services.
  • Accompany patients to appointments.
  • Educate patients about their medical condition, treatment plan, and available resources.
  • Maintain accurate and up-to-date records of patient interactions and services provided.

    Requirements

  • Bachelor's degree in healthcare administration, social work, psychology, or related field (preferred).
  • Previous experience in patient advocacy, healthcare coordination, or related role.
  • Strong interpersonal and communication skills, with the ability to build rapport and trust with patients.
  • Empathy, compassion, and a genuine desire to help others.
  • Excellent organizational skills and attention to detail.
  • Ability to work effectively in a fast-paced, dynamic environment.
  • Proficiency in Microsoft Office and electronic medical record systems.
  • Knowledge of healthcare systems and resources.
  • Bilingual proficiency (preferred).
  • All candidates must be able to successfully pass a CORI, reference and multi-state background check.
