Schedule : Mon-Fri 9am-5pm
Salary : $ / hour
We are seeking a compassionate and organized individual to join MMHC & Brigham and Women's Hospital's team as a Patient Navigator. As a Patient Navigator, you will play a crucial role in assisting patients through their healthcare journey, ensuring they receive the support and resources they need.
Responsibilities :
- Guide patients through the healthcare system, providing assistance and support at every step of their journey.
- Serve as a liaison between patients and healthcare providers, facilitating communication and ensuring patients understand their treatment options.
- Coordinate appointments, referrals, and follow-up care to ensure seamless healthcare delivery.
- Advocate for patients' needs and rights, ensuring they receive appropriate care and support services.
- Accompany patients to appointments.
- Educate patients about their medical condition, treatment plan, and available resources.
- Maintain accurate and up-to-date records of patient interactions and services provided.
