Rate: $44-48.00/hr
Are you detail-oriented, clinically sharp, and passionate about accuracy in patient care planning? A reputable skilled nursing facility is seeking an experienced RN MDS Coordinator to support resident assessments, care planning, and regulatory compliance. This is a key clinical role that directly impacts resident outcomes and the overall success of the care team.
The RN MDS Coordinator serves as the clinical lead for assessment accuracy and care planning. You will collaborate with nursing, rehab, and interdisciplinary teams to ensure complete, compliant documentation that supports quality outcomes and optimized reimbursement.
• Complete and manage timely, accurate MDS assessments per RAI process and CMS guidelines
• Lead and coordinate interdisciplinary care planning meetings
• Review documentation to ensure accurate coding for PDPM
• Monitor quality measures and contribute to QA initiatives
• Provide education and support to staff on documentation and care planning best practices
• Stay current with regulatory updates and ensure compliance with all requirements
• Active RN license NV
• 1+ year of MDS experience preferred
• Strong working knowledge of PDPM, RAI guidelines, care planning, and CMS regulations
• Exceptional attention to detail and organizational skills
• Ability to work collaboratively with clinical and administrative teams
• MDS/RAI certification preferred or willingness to obtain
