Position Summary :

The Pediatric Nurse Case Manager (NCM) has the primary responsibility of providing care to patients and families on Navian Hawaii's Pediatric Hospice Program.

Primary Responsibilities for Pediatrics :

• Works with the physician and the Interdisciplinary Group to assess, plan, implement and evaluate the plan of care for patients and their caregivers.
• Functions as primary nurse within the framework of an Interdisciplinary Group providing essential patient care.
• Utilizes principles of palliative care and holistic philosophy to provide holistic care.
• The NCM's plans reflect skilled nursing judgment, medical necessity, measurable patient / caregiver objectives and reasonable goals.
• Provides case management services to best benefit patients and caregivers.
• Care provided reflects compliance with federal and state regulations, along with Navian Hawaii's policies and procedures.
• Documents all nursing and case management activities in accordance with hospice protocol and keeps attending physician informed of changes in patient condition.
• Requests support services from other Interdisciplinary Group members as appropriate.
• Demonstrates good judgment in seeking and accepting guidance; facilitates clear and honest communication with other team members.
• NCM will ensure any follow up needs will be communicated to the after-hours team at the end of the day
