What You'll Do

As a Physical Therapist, you'll work 1 : 1 with patients in their homes, helping them regain strength, mobility, and confidence. You'll collaborate closely with physicians, caregivers, and a compassionate interdisciplinary team.

Key Responsibilities

Create individualized treatment plans based on physician orders

Provide goal-oriented physical therapy in patients' homes

Educate patients and families to support independence and recovery

Document all visits accurately and promptly in our EMR system

Attend interdisciplinary meetings to ensure coordinated care

Follow all agency policies and regulatory requirements

What You'll Need

Active Florida PT license in good standing

Current CPR certification

Level 2 background screening

Recent negative TB test or chest X-ray

Solid knowledge of physical therapy best practices

Great communication & teamwork skills

1+ year of PT experience (multidisciplinary setting preferred)

Experience with AlayaCare EMR is a plus
