Care Coordinator
In this role, you will collaborate with care teams to provide high-quality, patient-centered care while ensuring continuity and addressing care gaps for high-risk patients.
Key Responsibilities :
- Patient Outreach : Engage high-risk, post-discharge patients and schedule follow-up appointments with PCPs.
- Care Coordination : Work with care teams to manage patient records and track care gaps.
- Community Support : Connect patients with local resources to address their physical and psychosocial needs.
- Team Collaboration : Assist nursing, social work, and multi-disciplinary teams to enhance patient outcomes.
