Care Coordinator

In this role, you will collaborate with care teams to provide high-quality, patient-centered care while ensuring continuity and addressing care gaps for high-risk patients.

Key Responsibilities :

• Patient Outreach : Engage high-risk, post-discharge patients and schedule follow-up appointments with PCPs.
• Care Coordination : Work with care teams to manage patient records and track care gaps.
• Community Support : Connect patients with local resources to address their physical and psychosocial needs.
• Team Collaboration : Assist nursing, social work, and multi-disciplinary teams to enhance patient outcomes.

Qualifications :

1- 3 years in a medical office or healthcare setting.

Experience with Care Coordination

Preferred : 2 years as an LPN or MA or Pharmacy Tech

Technical Skills : Proficient with Microsoft Office and Electronic Health Records (EHR).

Training & Meetings :

On-Site Training : 3 weeks of training on-site.

Occasional On-Site Meetings : Periodic meetings will be held on-site.

Shift :

8-4pm

Monday-Friday

Remote : 100% remote work outside of training and limited on-site meetings
