About the Role
Are you passionate about precision medicine, cutting-edge therapies, and making a direct impact on patients facing complex hematologic and oncologic conditions? As the Clinical Pharmacist – Hematology/Oncology & BMT Specialist, you’ll be a trusted medication-therapy authority embedded within our Hematology/Oncology and Stem Cell Transplant (BMT) programs.
In this highly specialized role, you’ll lead the design, management, and optimization of advanced therapies—including chemotherapy, immunotherapy, cellular therapy (CAR‑T), and transplant-support medications—across inpatient and outpatient settings. You’ll work side by side with physicians, nurses, and interdisciplinary teams, driving safe, evidence-based, and innovative pharmacotherapy for some of our most complex pediatric patients.
This position blends clinical expertise, leadership, collaboration, and innovation, offering the opportunity to shape practice standards while mentoring the next generation of oncology pharmacy professionals.
What You’ll Do
Deliver Expert Clinical Care
- Optimize individualized medication regimens for hematopoietic cell transplant, CAR‑T, and bone marrow transplant patients.
- Participate in daily multidisciplinary rounds with ACGT, Oncology Infusion, and Inpatient Hematology/Oncology teams.
- Conduct continuous patient monitoring through chart review, clinical assessment, and real-time collaboration with care teams and families.
- Identify, prevent, and resolve complex drug-therapy problems to enhance safety and outcomes.
Advance Oncology & Transplant Pharmacy Practice
- Develop, implement, and maintain evidence-based chemotherapy protocols, treatment pathways, and clinical guidelines.
- Serve as a subject matter expert in hematology/oncology, CAR‑T, and BMT pharmacotherapy.
- Collaborate with the ACGT Medical Director to achieve and sustain FACT accreditation.
- Promote safe, effective, and cost-conscious medication use through formulary stewardship and patient education.
Lead & Mentor
- Oversee daily pharmacy operations across assigned units, including order verification and workflow coordination.
- Supervise, mentor, and train pharmacy technicians, interns, and pharmacists in oncology and transplant best practices.
- Ensure compliance with pharmacy SOPs, medication safety standards, and drug control regulations.
- Document clinical interventions and conduct monthly medication storage and preparation area inspections.
What You Bring
Required
- Bachelor of Science in Pharmacy
- Active Connecticut Pharmacist License
- Minimum 3 years of experience in hematology/oncology or an equivalent specialty
- Deep knowledge of medication indications, dosing, monitoring, contraindications, and drug interactions
- Strong critical-thinking, clinical judgment, and problem-solving skills
- Proficiency with EHRs and oncology pharmacy systems (., DoseEdge, Omnicell)
Preferred
- PharmD degree
- Hospital pharmacy experience
- Completion of PGY1 Pharmacy Practice Residency
- Completion of PGY2 Pediatric Pharmacy Practice Residency
- Board certification in a relevant specialty
- Advanced training or certification in hematology/oncology or BMT pharmacotherapy

Pay and BenefitsThe pay range for this position is $113000.00 - $175000.00/yr.
