Clinical Pharmacist candidates shall have the following job responsibilities :
The Clinical Pharmacist is responsible for developing and managing all Clinical Pharmacy Programs, including those required to maintain specialty pharmacy accreditation and credentialing
Maintains a strong knowledge and understanding of the company's specialty accreditation and credentialing requirements
Provides routine review updates to the clinical program in response to changes in accreditation and credentialing requirements and / or standards
Ensure compliance with all state and federal laws and regulations pertaining to the practice of pharmacy
Keep abreast of changing laws, regulations, and new pharmacy-related programs
Develops, performs, and maintains all aspects of clinical pharmacy programs according to policies and regulation.
Develops, writes, and implements policies related to clinical procedures, measures, and practices for presentation and approval by Pharmacy Quality Management Committee and Avita Leadership Committee
Provides supervisory oversight for the company's Medication Therapy Management program
Actively identifies practice related issues which require evaluation and facilitate clinical research projects, quality improvement initiatives, or healthcare provider education as needed to advance practice
Provides training to pharmacy and clinic staff for "best" and other adopted practices
Devises and implements medication therapy strategies for inclusion of clinic satellite locations in clinical pharmacy programs and requirements
Maintains strong working relationships with all AvitaCare Clinic staff members and Avita Pharmacy administration
Performs in accordance with company policies and standards, as well as the spirit of the company's core values and goals
Displays a high level of professionalism consistent with company standards, including dress and personal appearance
Perform other related duties as assigned
JOB QUALIFICATIONS-
Clinical Pharmacist
CLIN RX
Full-Time
Clinical Pharmacist candidates must meet the following minimum job qualifications :
Required : Pharm.D., or residency from an accredited institution.
Required : PGY 1 Residency in Hospital
Required : Licensed to practice in the State of Texas.
Required : A.C.P.E. accredited I.V. Certification, Completion of an accredited Residency in a Hospital Pharmacy (PGY1 / PGY2)
Required : BLS. Encouraged any Board of Pharmacy Specialty certifications.
Preferred : BCPS - Board Certified Pharmacotherapy Specialist or any board certifications.
Familiar with standard concepts, practices, and procedures within a particular field.
SHIFT : DAYS 8A-5P (MONDAY-FRIDAY, COVERAGE REQUIRED EVERY 3rd WEEKEND)
