Responsibilities :
Organize and conduct physical therapy programming for SSCHC to facilitate rehabilitation, which includes adapting programming or a plan of care according to the needs of the individual patient.
Direct patient participation in selected tasks to restore, reinforce, and enhance performance.
Facilitate the patients' learning of skills and functions that are essential for adaptation and productivity, to diminish or correct pathology.
Design and deliver effective and efficient rehab services which includes but is not limited to establishing an appropriate therapeutic regimen consistent with the needs and capabilities of each patient.
Make appropriate referrals to other disciplines or organizations to address the comprehensive needs of the patient.
Consult with other members of the rehabilitation team to select the most appropriate therapeutic regimen consistent with the needs and capabilities of each patient.
Screen, examine, and evaluate patients which will include but is not limited to taking medical history, conducting a review of systems, applying appropriate tests and measures, synthesizing information and interpreting examination findings to establish a diagnosis, identifying impairments, determining the predicted level of improvement and the time required to achieve it, and identifying precautions or contradictions to design an appropriate plan of care.
Develop appropriate treatment goals and methods in collaboration with the patient and caregivers, implement the physical therapy treatment plan, and complete all related documentation and record keeping.
Submit billing to the coding department.
Designs, fits, and applies adaptive devices and trains patients in the use of adaptive devices.
Instruct patient in the skills and techniques required for participation in functional activities and evaluates patient progress, attitudes, and behavior as related to rehabilitative potential.
Performs routine reexamination as needed to modify the progress plan of treatment.
Coordinate P.T. services with the medical department.
As necessary, provide supervision to the Physical Therapy Assistants.
Qualifications :
Licensed to practice as a Physical Therapist in the State of Wisconsin.
Must have a master's degree in physical therapy or must have a bachelor's degree in physical therapy plus 5 years of progressively responsible PT experience.
Ability to communicate effectively and educate patients.
Ability to establish and maintain effective working relationships.
BLS Healthcare Provider Certification.
Experience in Public Health and with multi-cultural populations preferred.
