Responsibilities :

• Organize and conduct physical therapy programming for SSCHC to facilitate rehabilitation, which includes adapting programming or a plan of care according to the needs of the individual patient.
• Direct patient participation in selected tasks to restore, reinforce, and enhance performance.
• Facilitate the patients' learning of skills and functions that are essential for adaptation and productivity, to diminish or correct pathology.
• Design and deliver effective and efficient rehab services which includes but is not limited to establishing an appropriate therapeutic regimen consistent with the needs and capabilities of each patient.
• Make appropriate referrals to other disciplines or organizations to address the comprehensive needs of the patient.
• Consult with other members of the rehabilitation team to select the most appropriate therapeutic regimen consistent with the needs and capabilities of each patient.
• Screen, examine, and evaluate patients which will include but is not limited to taking medical history, conducting a review of systems, applying appropriate tests and measures, synthesizing information and interpreting examination findings to establish a diagnosis, identifying impairments, determining the predicted level of improvement and the time required to achieve it, and identifying precautions or contradictions to design an appropriate plan of care.
• Develop appropriate treatment goals and methods in collaboration with the patient and caregivers, implement the physical therapy treatment plan, and complete all related documentation and record keeping.
• Submit billing to the coding department.
• Designs, fits, and applies adaptive devices and trains patients in the use of adaptive devices.
• Instruct patient in the skills and techniques required for participation in functional activities and evaluates patient progress, attitudes, and behavior as related to rehabilitative potential.
• Performs routine reexamination as needed to modify the progress plan of treatment.
• Coordinate P.T. services with the medical department.
• As necessary, provide supervision to the Physical Therapy Assistants.
