What You Will Do:

  • Lead the day-to-day clinical and operational activities of BHN's Program of Assertive Community Treatment team.
  • Coordinate admissions and the delivery of treatment, rehabilitation, and support services in partnership with the program psychiatrist and multidisciplinary team.
  • Develop staffing schedules that ensure appropriate coverage across day, evening, weekend, holiday, and on-call hours.
  • Lead daily organizational and treatment-planning meetings while continuously evaluating changing needs and coordinating timely clinical responses.
  • Oversee comprehensive assessments, individualized treatment plans, risk plans, crisis plans, progress notes, and other required clinical documentation.
  • Provide comprehensive intake, risk, and crisis evaluations and take appropriate action to support the safety of individuals served and others.
  • Provide regular individual and team supervision, ensure staff competency in the PACT model, and support ongoing professional development and performance.
  • Ensure compliance with PACT, agency, funding-source, third-party, licensing, and Medication Administration Program policies and standards.
  • Coordinate care with families, residential providers, hospitals, community partners, funding sources, and other BHN programs to support continuity and community integration.
  • Participate in the program's 24/7 management on-call rotation and provide services across homes, hospitals, and other community settings as needed.

Who You Are:

  • Master's degree in Social Work, Psychology, Mental Health Counseling, or a related clinical field required.
  • Current clinical licensure required.
  • Previous supervisory experience required; at least two years of human-services experience preferred.
  • Commitment to individual empowerment, personal growth, human rights, dignity, safety, and community integration.
  • Ability to support individuals with complex and diverse needs, including those involved with the forensic system.
  • Working knowledge of community resources, crisis response, risk assessment, treatment planning, and the PACT model of care.
  • Strong clinical judgment, cultural responsiveness, communication, organization, collaboration, and conflict-resolution skills.
  • Valid driver's license and reliable vehicle available during working hours to support community-based services and transportation needs required.
