What You'll Do
Position Summary
The Neighborhood Center Advanced Practice Provider serves as a health-care professional who drives, supports, diagnose, and cares for a variety of health-related problems for Suvida Healthcare's patients. The provider will manage these patients with the supervision of the Neighborhood Center Medical Director in a multidisciplinary value-based care model. The provider work assignments involve low-emerging risk complex patients to high complexity patients where the analysis of situations or data requires an in-depth evaluation of variable factors. The clinician will collaborate with a multidisciplinary care team and will serve as the leader of their respective care team within their Neighborhood Center.
