The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Psychiatric Nurse Practitioner works closely with Primary Care Medical Providers and Behavioral Health Providers to support patients with severe and ongoing mental health needs. This role provides psychiatric evaluations, medication management, and follow-up care, primarily at the Stout Street Health Center. The position focuses on delivering integrated, trauma-informed care to individuals experiencing homelessness or housing instability and may also provide services across other Colorado Coalition for the Homeless programs, including Housing Support Services, Street Outreach, and Recuperative Care.

NOTE : In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date.  The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance.  Please note that a start date will be scheduled once credentialing and privileging is complete.

  • Valid driver’s license required. This requirement may be waived, if necessary, based on overall candidate experience and current needs of the business.

    Essential Job Functions

  • Provides direct psychiatric care, including new patient evaluations, follow-up care, and care coordination on both a scheduled and walk-in basis.
  • Collaborates closely with Behavioral Health Providers (BHPs) to provide psychiatric consultation to Primary Care Medical Providers (PCMPs).
  • Provides urgent and crisis psychiatric care as coordinated by BHPs.
  • Conducts diagnostic assessments consistent with DSM-5 and / or ICD-10, taking into account patients’ economic and housing circumstances, medical conditions, and substance use disorders.
  • Orders and interprets diagnostic tests as clinically indicated.
  • Develops treatment recommendations addressing both mental health and substance use disorders, considering patients’ trauma histories, economic conditions, and housing status.
  • Creates individualized treatment plans that align with the patient's goals and objectives.
  • Coordinates care with other providers within CCH and external partner agencies.
  • Collaborates with colleagues on complex cases; refers patients for consultation or transfer of care when treatment reaches an impasse or when acuity exceeds the scope of practice.
  • Works collaboratively with all members of the healthcare team to provide comprehensive, integrated care.
  • Maintains a respectful, inclusive work environment that supports workforce diversity, cooperation, and teamwork.
  • Evaluates patients' psychiatric disability status and completes required reports and documentation.
  • Encourages client engagement in treatment by offering information and choices in a respectful, non-judgmental manner.
  • Participates in regular team meetings, including weekly suite meetings, bi-monthly discipline meetings, and other scheduled trainings.
  • Contributes to initiatives aimed at improving access to compassionate, efficient, and user-friendly healthcare delivery.
  • Practices in alignment with CCH’s mission and Philosophy of Service, incorporating harm reduction, motivational interviewing, and recovery-oriented care.
  • Participates in assessments of patient risk and facilitates transfer to inpatient care when clinically indicated; may initiate Mental Health Holds when appropriate.
  • Documents all patient care activities using the Electronic Health Record (EHR), including :
  • Recording clinical services in a timely and complete manner
  • Maintaining up-to-date charts and records
  • Documenting telephone and other communications as needed
  • Managing tasks, PAQs, and scanning for a complete chart
  • Maintaining accurate insurance information and completing CCARs, DACODs, and other authorizations as required
  • Utilizing the EHR and implementing future upgrades as directed
  • Maintains and enhances professional knowledge and strives to continuously improve clinical care delivery.
  • Adheres to HIPAA and professional standards regarding patient confidentiality.
  • Engages in quality improvement efforts, outcome evaluations, and peer review activities.
  • Participates in regular supervision with the Psychiatric Director or designee.
  • Performs other duties as assigned.
  • Qualifications Summary

  • Preference for candidates whose past experience includes significant psychiatric experience.
  • Proven clinical competence in the provision of psychiatric health care in a community setting.
  • Demonstrated experience, competence, and sensitivity in working with homeless and at-risk individuals and families.
  • Education

  • A Master of Science degree in Nursing from a program accredited by the American Nurses Association or the American Association of Nurse Practitioners.
  • Current registration in the State of Colorado as an Advanced Practice Nurse.
  • Certification as a Psychiatric Nurse Practitioner from the American Nurse’s Credentialing Center.
  • Prescriptive authority from the Colorado State Board of Nursing. Consideration will be given to candidates who have completed all the requirements for this authority except for supervisory hours.
  • Graduation from a program accredited by the American Academy of Physician Assistants.
  • Current licensure with the State of Colorado to practice as a Physician Assistant.
  • Current certification with the National Commission of Certification of Physician Assistants.
  • Certificate of Added Qualification in Psychiatry required.
  • WHERE A CANDIDATE IS PLACED IN THE COMPENSATION RANGE DEPENDS ON TOTAL RELEVANT YEARS OF EXPERIENCE

    Qualified candidates are eligible for the  NURSE Corps Loan Repayment Program  which offers   payment of  60% of unpaid nursing education debt  in exchange for a two-year commitment at a NURSE Corps approved site  with an option to extend to a third year for an additional 25% of the original balance.
