Job Summary
Join our dedicated team as a Field Nurse Practitioner, where you will provide essential screening, preventive primary care, and medical services to our members in comfortable non-clinical settings like homes, community spaces, nursing facilities, and 'pop-up' clinics. Your role will be vital in helping members achieve their health goals while contributing to an overarching strategy aimed at enhancing quality and cost-effective care.
Essential Job Duties
- Deliver general medical care and coordination for various patient populations, including adults, women, children, and seniors.
- Conduct thorough evaluations, including history and physical exams, to identify care gaps and preventive needs.
- Effectively address both chronic and acute primary care concerns while recognizing medical urgency.
- Establish and document accurate medical diagnoses.
- Request specialty consultation as necessary.
- Order and perform relevant diagnostic tests judiciously to aid in diagnosis and treatment.
- Determine when a member requires further intervention by a physician and facilitate that process.
- Create and implement tailored medical plans of care.
- Coordinate post-discharge follow-up to minimize hospital readmissions and emergency visits.
- Provide in-person visits across diverse settings, including private homes, skilled nursing facilities, and community locations.
- Utilize alternative modalities for visits as dictated by business needs and within legal frameworks.
- Manage bulk laboratory testing aimed at specific member populations.
- Participate in on-call duties to triage urgent lab results and pharmacy inquiries.
- Engage with the community through participation in 'pop-up clinics' to address healthcare gaps.
- Regularly travel up to 120 miles within your assigned region, with potential additional travel on special project days.
- Obtain and maintain multi-state licenses as required for your role.
- Collaborate with fellow practitioners to establish efficient best practices.
- Engage actively in regional meetings.
- Possibly prescribe medications and perform relevant procedures.
- Document medical records promptly using our electronic medical record (EMR) system.
- Lift and transport equipment weighing up to 50 lbs. and navigate stairs as needed.
- Work in compliance with medical practice laws under the supervision of a licensed physician.
