Amergis Healthcare Staffing is seeking a Disease Management Care Coordinator (DMCC) to assess member's current health status and understanding of their chronic condition. The DMCC provides education to assist with self-management, provides information on network access, and identifies and implements interventions and resources that address and close gaps in care.

Minimum Requirements :

• Current / active professional nursing license required
• Three (3) years of clinical experience within managed care environment preferred
• Must be at least 18 years of age
