Job description

Registered Nurse - Home Health
Location: Sterling Heights, MI
Salary Range: $62,000 to $80,000 per year
Position Summary
A compassionate and skilled Registered Nurse is being sought to join a growing home health team based in Sterling Heights, Michigan. This is an exceptional opportunity for a dedicated nursing professional who thrives in an autonomous, patient-centered environment and is passionate about delivering high-quality clinical care directly to patients in their homes. Home health nursing represents one of the most rewarding and rapidly expanding sectors within the broader healthcare industry, offering nurses the unique ability to build meaningful, ongoing relationships with patients and their families while making a genuine difference in everyday lives.
In this role, you will manage a caseload of patients across the Sterling Heights and surrounding metro Detroit communities, providing skilled nursing services that support recovery, chronic disease management, and overall wellness. You will work closely with interdisciplinary care teams including physicians, therapists, social workers, and care coordinators to develop and implement individualized care plans tailored to each patient's specific medical needs and personal goals. As the home health industry continues to grow in response to an aging population and increased preference for receiving care at home, qualified nurses in this field are in high demand. This position offers strong career advancement potential, professional development support, and the chance to work within a clinical culture that deeply values both patient outcomes and employee well-being.
Core Responsibilities

  • Conduct comprehensive initial assessments and regular reassessments of patients in their home environments
  • Develop, implement, and continuously update individualized care plans in collaboration with the multidisciplinary care team
  • Administer medications, wound care, infusion therapy, and other skilled nursing interventions as ordered
  • Monitor and evaluate patient response to treatment plans and communicate changes to the supervising physician promptly
  • Educate patients and family members on disease management, medication adherence, safety practices, and self-care techniques
  • Accurately document all clinical activities using an electronic health record system in compliance with regulatory requirements
  • Coordinate with discharge planners and case managers to ensure smooth transitions of care
  • Participate in on-call rotations as required to support continuous patient care coverage
  • Maintain compliance with all applicable state, federal, and accreditation standards governing home health practice

Must-Have Requirements

  • Active and unrestricted Registered Nurse license in the state of Michigan
  • Minimum of one year of clinical nursing experience, with home health or medical-surgical experience strongly preferred
  • Current CPR and BLS certification
  • Valid Michigan driver's license and reliable personal transportation
  • Willingness and ability to travel to patient homes throughout the Sterling Heights and greater metro Detroit area
  • Strong clinical assessment and critical thinking skills
  • Proficiency with electronic health record documentation systems
  • Excellent communication, time management, and organizational abilities
  • Ability to work independently while maintaining strong collaboration with a care team

Benefits and Perks

  • Competitive salary ranging from $62,000 to $80,000 annually, commensurate with experience
  • Comprehensive medical, dental, and vision insurance coverage
  • Employer-contributed retirement savings plan
  • Generous paid time off including vacation, sick leave, and observed holidays
  • Mileage reimbursement for patient-related travel
  • Employer-provided or reimbursed cell phone and technology support
  • Continuing education reimbursement and professional development opportunities
  • Tuition assistance for nurses pursuing advanced degrees or specialty certifications
  • Flexible scheduling options to support work-life balance
  • Supportive clinical leadership and a collaborative, team-oriented work culture
