MDS Coordinator Job Description
Join Our Team at Aberjona Rehabilitation and Nursing Center!
We are seeking a dedicated and experienced MDS Coordinator to join our care team at Aberjona Rehabilitation and Nursing Center, a 123-bed facility that provides short-term rehabilitation, skilled nursing services, and long-term care, with a Joint Commission accreditation with commendation. If you are passionate about delivering exceptional patient care and want to be part of a collaborative and supportive environment, we invite you to apply for this full-time position.
Responsibilities:
Conduct and coordinate the development and completion of the resident assessment
Conduct resident assessments in accordance with current rules, regulations, and guidelines
Implement resident CAA's
Participate in facility survey readiness and throughout survey
Work with our interdisciplinary care team in developing a comprehensive assessment and care plan for each resident
Qualifications:
Current unencumbered State of Massachusetts RN or LPN License
Two years of MDS and PDPM experience
Care Planning experience
