Job descriptionDietitians registered after 1 / 1 / 2024 must hold a Master's Degree and accredited Dietetic Internship. 3 years experience in an acute care setting with recent hospital experience-OR previous clinical nutrition practice experience with a minimum of 5 years, which includes working with a multidisciplinary health care team Registered Dietitian (RD), Registered Dietitian Nutritionist (RDN) Upon start New hires within 3 months Current TMs within 6 months of 01 / 18 / 2023 Performs a comprehensive nutrition assessment by analyzing and interpreting data from multiple sources. Formulates an individualized nutrition plan of care based on assessment findings and interpreting trends in patient populations. Coordinates the nutrition plan of care with the physician and other healthcare providers while serving as a resource in the integration of the nutrition plan of care into the comprehensive plan of care for patients. Advocates for patients rights from the family / patients perspective while providing counsel and acting as a resource to resolve issues. Provides appropriate documentation in the medical record summarizing the nutrition plan of care including assessment, diagnosis, plan, implementation and progress toward goals. Documents all patient care activities, as per documentation standards, and assists healthcare team members with documentation. Other duties as assigned.
The Dietitian Registered Team Lead provides safe and therapeutic nutritional care in a comprehensive and systematic manner. Acts as a resource to physicians, nurses and other allied professional healthcare team members to provide patients with optimum nutritional care. Ensures that improvements in practice settings occur by assuming responsibility for self-development and the development of staff in lifelong learning. Provides direction and guidance for others regarding practice while serving as a resource, preceptor and mentor.
