Director of Rehabilitation (DOR)

Position Summary
We are committed to delivering exceptional, patient-centered care in a dynamic and supportive environment. The Director of Rehabilitation (DOR) serves as the clinical and operational leader of the therapy department, overseeing physical therapy, occupational therapy, and speech-language pathology services.

This leadership role is open to qualified clinicians from Physical Therapy, Occupational Therapy, or Speech-Language Pathology, bringing diverse clinical perspectives to drive excellence in care.

The DOR is responsible for advancing clinical quality, developing innovative rehabilitation programs, ensuring regulatory compliance, and fostering a collaborative, high-performing team culture dedicated to outstanding patient outcomes.

Key Responsibilities

Leadership & Program Development

Lead, mentor, and supervise all rehabilitation staff across PT, OT, and Speech Therapy
Develop and implement high-quality, outcomes-driven rehabilitation programs in collaboration with leadership and medical staff
Champion innovative programming, including restorative nursing and specialty rehab pathways (e.g., orthopedic, cardiac)
Integrate rehabilitation services into the overall resident care experience

Operations & Compliance

Partner with regional and corporate leadership to manage staffing, budgeting, and departmental performance
Ensure compliance with federal, state, and professional practice standards
Maintain and update departmental policies and procedures
Resolve operational challenges and escalate issues when appropriate

Clinical Oversight & Quality Assurance

Conduct monthly QA reviews and chart audits
Ensure accuracy and compliance of clinical documentation
Address deficiencies through education and coaching
Lead quarterly program evaluations and implement improvements

Technology & Performance Management

Utilize Net Health (formerly Optima) to monitor therapy delivery, outcomes, billing, and compliance
Track key metrics including productivity, length of stay, outcomes, and census
Leverage dashboards to monitor compliance, PDPM performance, and operational efficiency

Staff Development & Culture

Oversee onboarding, training, and competency evaluations
Identify education needs and coordinate in-services
Foster a positive, accountable, team-oriented culture

Admissions, Discharges & Program Evaluation

Review referrals and admissions for appropriate service utilization
Evaluate discharges and outcomes to improve care delivery
Assess program effectiveness and recommend enhancements

Communication & Leadership Presence

Provide regular updates to the Administrator
Present QA and operational reports
Serve as a key member of the interdisciplinary leadership team

Marketing & Growth

Support census growth through screenings, outreach, and referral development
Contribute to marketing efforts highlighting program success
Participate in market analysis and program development

Patient Care Responsibilities

Provide direct patient care as needed
Maintain discipline-specific clinical standards

Schedule Expectations

May include weekend coverage based on operational needs

Qualifications

Bachelor’s or Master’s Degree in Physical Therapy, Occupational Therapy, or Speech-Language Pathology
Current or eligible state licensure
Minimum of 3+ years of rehabilitation experience
Proven leadership experience in a healthcare setting
Strong understanding of SNF operations, PDPM, and regulatory compliance
Excellent communication, organizational, and problem-solving skills
Skilled Nursing Facility experience preferred

Compensation & Benefits

Salary: $125,000 – $150,000 annually
