Job description

Medical Assistant - Population Health Central Valley

The Population Health Medical Assistant supports primary care teams within the network, either remotely or embedded in a primary care office, to enhance compliance with population health initiatives such as annual wellness visits, quality care gap closure, and in-network referrals. This role does not involve direct patient care but requires executing standing orders for patients.

Responsibilities

  • Regularly review patient charts for screening needs and quality gap closure, as well as care coordination needs.
  • Work under appropriate physician licensure to use standing orders developed by the primary care physician for screening and quality gap closure needs.
  • Perform chart preparation prior to office visits to identify open care gaps.
  • Manage in-network referrals for value-based patients.
  • Conduct member outreach to schedule patient visits for campaigns like annual wellness visits and cancer screenings.
  • Coordinate follow-up visits for transitions of care.
  • Transfer patient data regarding quality care gaps from clinic medical records to population health management platforms.
  • Proactively address payer care gaps and frequent flier/high-risk reports.
  • Collaborate with primary care teams and clinical performance consultants to manage value-based patients effectively.
  • Fully disclose relevant training, experience, and credentials to help patients understand the scope of services provided.
  • Work collaboratively with the team in the physician office and assigned support teams.
  • Understand ALOS, readmission rates, and Gaps in Care.
  • Other duties as assigned by the supervisor.

Essential Skills

  • Proficiency in electronic health records and appointment scheduling.
  • Experience with EMR systems.
  • At least three years of experience as a Medical Office Referral Coordinator, Medical Assistant, or similar roles.
  • Diploma from a Medical Assistant program.
  • Familiarity with CPT codes, especially CPT-2 codes.
  • Experience with Microsoft Office Suite, including Outlook, Calendar, and Excel.
  • Experience with quality measures like care gap closures, pre-visit planning, HEDIS measures, and patient outreach.
  • Excellent verbal communication skills.

Additional Skills & Qualifications

  • Ability to follow instructions, analyze data, coordinate tasks, and manage workload to meet business goals.

Work Environment

The position is based out of a central hub and requires floating to different provider offices across Maricopa County. On Mondays, work from the office in Arcadia. From Tuesday to Thursday, work from clinics within the Central Phoenix / Tempe territory will cover Central Maricopa County area. Fridays may involve working at a clinic or from home, depending on clinic hours. Must have reliable transportation, be reliable and be able to work an 8 hour shift M-F 6a-6p.

This is a Contract to Hire position based out of Tempe, AZ.

Pay and Benefits

The pay range for this position is $22.00 - $26.00/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment. Benefits are subject to change and may be subject to specific elections, plan, or program terms.
