DESCRIPTION
Responsible for having a broad understanding of the services provided by PIHC.
Routinely reviews and utilizes patient-level data to identify PIHC patients with poor health outcomes and / or at-risk of falling out of care. Works closely with clinic, medical case management and behavioral health staffs to resolve patient barriers to care.
Works Primarily at the Duluth location and visits the Decatur and Marietta locations weekly.
Attends inter-agency meetings, workshops and required agency trainings.
Helps drive and maintain the agency van at the Duluth location. Has a valid, GA Driver's license
Organizes and schedules all transportation requests for the Duluth location and assists with transportation requests for the Marietta and Decatur location.
Maintains excel spread sheets for tracking grant requirements for transportation funding (RW Part A, Kaiser, 340B, Cobb County ARPA Funding and HOPWA).
Enters Transportation data in client’s Electronic Medical Records weekly.
Tracks and makes sure that client's Annual Non-Emergency Transportation waivers are in compliance and deemed correct.
Possess and demonstrate excellent customer service skills as well as excellent Interpersonal and organizational skills.
Tracks and records MARTA Breeze Cards and Quik Trip cards.
Manages the Transportation Pool in Cerner
Requires travel to all 3 PIHC locations : Duluth, Decatur and Marietta.
