Must Haves :

Case Management Experience

Current, unrestricted RN license in the state of employment

Background in managed care

BLS or ACLS

3+ years of direct patient nursing care with a focus on discharge planning or case management

Ability to type and navigate a Windows-based environment

Experience or exposure to discharge planning

Bachelor’s degree in Nursing

Plusses :

Experience in utilization review, concurrent review, or risk management.

• Previous experience in a telephonic role.

Knowledge of chronic diseases (COPD / asthma, diabetes, CHF, IHD).

Day to Day : As a Case Manager RN, your day-to-day responsibilities will involve working closely with providers and clinic staff to identify high-risk patients and support their chronic care needs. You’ll perform health condition assessments, conduct medication reconciliations, and use motivational interviewing and self-management goal setting to provide patient education and create referrals to appropriate agencies and resources. Transition support is a key part of your role, where you’ll make outbound and receive inbound calls to assess members’ health status, identify gaps or barriers in treatment plans, and assist with post-discharge needs such as prescriptions, transportation, and durable medical equipment. You’ll coordinate with providers to update care plans, perform accurate and timely documentation in the electronic medical record, and participate in daily huddles and Patient Care Coordination (PCC) meetings. Additionally, you’ll prepare reports for weekly meetings, maintain competence in nursing practice and current evidence-based practices, and may perform clinical tasks within your scope of practice. Other duties will be assigned as needed to ensure comprehensive patient care.
