The RN will work closely with physicians, nurse practitioners, dietitians, and other healthcare professionals to create personalized care plans and ensure the highest quality of care for patients with diabetes. This position requires strong clinical knowledge of diabetes management, patient education, and the ability to provide empathetic and compassionate care to individuals with chronic conditions.Major Responsibilities
- Patient Assessment & Care : Perform initial and ongoing assessments of diabetic patients, including vital signs, blood glucose levels, and relevant medical history. Develop individualized care plans based on assessments and collaborate with the healthcare team for ongoing patient management.
- Patient Education : Educate patients and their families on diabetes self-management, including blood glucose monitoring, insulin administration, diet, exercise, and medication adherence. Provide resources to promote healthy lifestyle changes and disease prevention.
- Diabetes Management Support : Assist with insulin injections, insulin pump management, continuous glucose monitoring (CGM), and other diabetes-related therapies. Support patients in achieving optimal blood glucose control and minimizing complications.
- Chronic Disease Management : Monitor for potential complications of diabetes, such as neuropathy, retinopathy, and nephropathy, and refer patients to specialists as needed. Implement interventions to manage co-existing conditions like hypertension, hyperlipidemia, and obesity.
- Patient Advocacy : Provide emotional support and counseling to patients coping with the challenges of living with diabetes. Advocate for patients' needs within the healthcare system.
- Collaboration with Healthcare Team : Collaborate with physicians, nurse practitioners, dietitians, and other professionals to develop and implement individualized care plans. Participate in interdisciplinary meetings to review patient progress and adjust care plans as necessary.
- Documentation & Reporting : Maintain accurate, up-to-date patient records, including progress notes, medication updates, and changes in care plans. Ensure compliance with all healthcare regulations and institutional protocols.Required Qualifications
- Current Registered Nurse (RN) license in the state of Louisiana.
- Associate's or Bachelor's degree in Nursing (ADN or BSN) from an accredited nursing program.
- Minimum of two (2) years of nursing experience, with at least 1 year of experience in diabetes care or management in a clinical setting.
