ESSENTIAL DUTIES AND RESPONSIBILITIES
- Provide clinical consultation regarding complex healthcare issues or newly diagnosed conditions that require an in depth healthcare management plan.
- Review Physician Orders, including treatments and medications with the Residential Manager and / or Program Director.
- Periodic review of Individuals dietary restrictions and residential menus with Residential Managers
- Follow hospital admissions and be the liaison between RTR and DDS for all medical information.
- Assist RTR Management with discharge planning.
- Review DDS Health Screen Recommendations with the Residential Managers and / or Program Directors to provide all individuals with healthcare services in accordance to DDS regulations.
- Attend DDS monthly meetings and / or other required meetings concerning healthcare planning.
- Attend family meetings concerning individuals healthcare / medical issues.
- Provide as requested communication concerning medical issues to all ISP team members.
- Obtain MAP Certification.
- Assist RTR Management in Medication monitoring.
- Provide staff training as necessary.
- Participate in on-the-job training and provide coaching to new co-workers as directed.
- Adhere to agency safety standards.
- Perform additional duties, or substitute for others as required. Adhere to all RTR policies and procedures.
- Develop and provide all services in accordance with RTR's philosophies including staff and Board beliefs.
QUALIFICATIONS
LANGUAGE SKILLS
Must be fluent in English and have the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or government regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to communicate effectively with individuals under supervision, employees of the organization, families, and members of the community.
