Description :

Responsible for providing direct nursing care as ordered by physician, outlined in the plan of care, and in accordance with standards of nursing practices and regulations. Takes charge in leading and directing resident / patient care on assigned shift. Directly supervises Licensed Practical Nurses and Certified Nursing Assistants to ensure assignments and care are complete based on professional standards.

Requirements :

Personally participates in the assessment and delivery of care including medication administration, treatments, Adl care, etc. as ordered by physician and outlined in the plan of care. Performs all direct care outside of LPN scope of practice while remaining within RN scope of practice.

Conducts regular rounds to monitor resident activity and ensure resident / patients receive quality care based on professional standards and facility policies

Assesses residents physical and psychosocial status

Communicates with residents, family members, physicians and supervisor regarding resident status.

Ensures the medical records are maintained up to date and accurate during assigned shift

Ensures all documentation assigned for clinical staff is complete prior to the end of the shift

Coordinates and prepares for new admissions, transfers, discharge of residents

Transcribes physician orders

Assists in updating the plan of care that addresses the needs of the residents / patients

Completes incident / accident reports

Understands, complies with and promotes all rules regarding resident rights

Maintains safe and clean work area

Ensures proper storage of equipment and supplies

Ensures Infection prevention and control practices are followed on assigned unit

Reports hazardous conditions, incidents and accidents, and allegations to Director of Nursing and / or Administrator

Ensures ordered diagnostic test are completed and followed up

Completes scheduled assessments

Delegates appropriate duties assignments to Charge nurses and Certified Nursing Assistants
