Our Hospice House in Bend is a specialty inpatient, acute care hospice facilityan oasis of comfort and peace for patients and their families. Status : Full-time, hourly, non-exempt. 40 hours per week.
Schedule : Monday-Friday, 8 : 00 AM to 5 : 00 PM. Work outside of these hours may be required.
Location : Working all throughout Central Oregon in patients' homes and facilities.
Pay : $34.00-51.00 per hour, based on experience.

Summary / Objective :
The Social Worker provides comprehensive counseling and related support services to assist in meeting the identified psychosocial needs of patients and families enrolled in hospice, home health, palliative care and other Partners In Care programs / services. This role ensures that patients and families receive palliative care that meets the standards of Partners In Cares mission statement, policies, and procedures. The Social Worker offers psychosocial and emotional support and engages in interdisciplinary collaboration in both community-based and facility settings.
