The Social Services Director is a pivotal leadership role responsible for overseeing the social services department and ensuring the highest quality care for our residents. As a key member of our team, you will be responsible for developing and implementing social services programs, managing caseloads, and coordinating with healthcare professionals to provide comprehensive care. Responsibilities:
Develop and implement social services programs that cater to the needs of our residents, including counseling, case management, and discharge planning
Manage a caseload of residents, providing individualized support and guidance to ensure their social and emotional well-being
Collaborate with healthcare professionals to develop and implement comprehensive care plans that address the physical, emotional, and social needs of our residents
Coordinate with community resources and outside agencies to provide additional support services to residents
Participate in multi-disciplinary care conferences to ensure seamless care and communication among team members
Recruit, train, and supervise social services staff to ensure excellent service delivery
Monitor and evaluate program effectiveness, making recommendations for improvements
Develop and maintain relationships with community partners and stakeholders
Requirements:
A Master's Degree in Social Work
Bilingual Spanish - preferred
Experience managing Social Service Aides
Experience working with residents in a skilled nursing facility with behavioral/psychiatric needs or diagnoses
