Assist in conducting social assessments of residents to identify their emotional, social, and psychological needs. Offer support to residents and their families during transitions into long-term care, including coping with changes in living conditions and health status. Assist in the development, implementation, and monitoring of individualized care plans. Maintain accurate, up-to-date records of resident interactions, assessments, and case notes. Prepare reports for regulatory agencies, as needed. Act as an advocate for residents' rights, ensuring their voices are heard in care planning meetings. Assist families in understanding available services, including financial assistance, and help with navigating the healthcare system. Help connect residents and their families with community services and resources Coordinate with external agencies, healthcare providers, and social service organizations to ensure the residents' needs are met. Assist in identifying and managing emotional or behavioral crises among residents. Provide de-escalation techniques and support in high-stress situations. Ensure all interactions and activities comply with facility policies and regulations, including HIPAA and other confidentiality standards. Support quality assurance initiatives, ensuring the social services department meets organizational and regulatory standards. Bachelor's degree in Social Work or related field Must have an LSW Long term care experience preferred
