Amergis Healthcare is seeking an LPN In Home Case Manager to provide admissions assessments in Cambridge and the surrounding area. In this fulfilling role, you will provide community-based care services designed to help elderly and disabled individuals remain safely and comfortably in their homes.
Local Pay : $47 / hour + mileage reimbursed at $0.70 / mile
Qualifications :
- Active, unrestricted MA nursing license
Schedule : Monday-Friday, 9 : 00 AM-5 : 00 PM
Job Duties :
Complete intake, annual assessments, and admission visits.
Complete all required documentation, including client assessments and multidisciplinary plans of care; coordinate with PCPs and other healthcare providers.
Review and educate caregivers on admission binder materials, communication tools, and / or EVV systems.
Conduct regular in-home visits, provide caregiver skills training, and assess for health or environmental changes.
Conduct semi-annual visits, updating care plans, goals, medication reviews and reconciliations, and caregiver communication tool audits.
Identify concerns related to safety, psychosocial status, and physical health.
Duration : 13 weeks with possible extension
Location : Cambridge, Arlington, Somerville, East Boston, Newton, Wellesley, and Needham, MA
