PURPOSE AND SCOPE :
Supports FMCNAs mission vision core values and customer service philosophy. Adheres to the FMCNA Compliance Program including following all regulatory and division / company policy requirements. Works closely with VP Social Work Services to develop plan communicate and drive nationwide execution of key FMS clinical initiatives related to social work services.
Provides insight analysis support and strategic recommendations for the development dissemination and implementation of the various social work services programs and initiatives ensuring alignment with company goals and strategies and compliance with all pertinent regulatory requirements. Provides leadership and oversight to ensure successful and timely execution of the project and programs related to social work services initiatives. Communicates and drives the implementation of social work services policies strategies and initiatives. Develops and implements training and education programs.
PRINCIPAL DUTIES AND RESPONSIBILITIES :
Strategically drives and directs national social work services program development and implementation providing guidance support and expertise regarding specific assigned social work services projects under the direction of the VP of Social Work Services to ensure consistent application of strategies initiatives and programs.
Participates in the development of departmental strategic plans goals and objectives ensuring alignment with operation and medical office goals. Implements disseminates and administers operations strategies and objectives to ensure the achievement of the goals and objectives
Collaborates with Clinical Quality leadership regarding establishing operational objectives and work plans based on business goals to assure alignment. Communicates regularly with other teams such as nutritional services quality education regulatory clinical services and operations obtaining feedback and analyzing the recommendations made to contribute to the resolution of related issues and create positive outcomes.
Drives quality improvement initiatives / activities throughout FMS utilizing Social Work Services Managers to achieve standards identified in CMS Conditions for Coverage and FMS quality requirements. Evaluates satisfaction results and initiates / drives improvement activities as needed.
Technically proficient in utilizing appropriate dashboards to monitors high-level quality trends. Analyzes complex quality issues using in-depth knowledge of organizational objectives. Evaluates results and identifies strategic opportunities for Social Work Services Managers to drive quality improvement activities.
Responsibilities include but are not limited to evaluating best practices specifically regarding social work services pertaining to clinical initiatives research quality improvement and development of ideas to create strategies for implementation process identification and planning evaluation of the implementation and adoption of clinical initiatives / projects and technology systems.
Serves as a subject matter expert providing clinical guidance general direction and leadership in the area of social work services. Ensures the consistent application of quality social work service standards in support of excellent social work care of dialysis patients throughout their assigned business unit.
Responsible for and achievement of the social work-related value based care key performance indicator within assigned territory.
Responsible for and achievement of Quality Incentive Program (QIP) social work-related key performance indicators such has health equity and depression screening.
Acts as a Social Work Services representative on special cross-functional workgroups and company initiatives as assigned by VP of Social Work Services. Broad latitude for decisions within scope of the job. Decisions are usually complex and have an impact on the business beyond the functional area with serious impact on the overall success or failure in the area of accountability. Erroneous decisions will have a serious impact on the overall success of functional division or company operations.
Collaborates with Social Work Services leadership to develop and implement patient social work educational programs and material for home and in-center patients aimed towards improving patient experience increasing patient understanding of disease related social work modifications and improving patient quality outcomes.
Evaluates and interprets existing and emerging clinical indicators to improve delivery of social work services consistent with patient care standards and company goals.
Ensures a strong and robust communications process between all managers and facility social workers within the operating groups as needed by leading communications sessions and meetings with managers and ensuring the appropriate dissemination of communications
Collaborates with the VP Social Work Services regarding establishing operational objectives and work plans based on business goals to assure alignment. Communicates regularly with other teams such as quality and operations obtaining feedback and analyzing the recommendations made to contribute to the resolution of related issues and create positive outcomes.
Interacts with all levels within the organization to manage functional area. Overall responsibility for hiring coaching and counseling employees including performance reviews disciplinary action and terminations.
Ensures that budgets and schedules are within company requirements with significant organizational responsibility for the overall control of planning staffing budgeting expense priority management and recommendation and implementation changes of current methods
Provides leadership guidance and coaching for all direct reports to maintain an engaged and productive workforce; partnering with Human Resources on employee matters.

EDUCATION AND REQUIRED CREDENTIALS :
Masters Degree in Social Work required
EXPERIENCE AND SKILLS :
5 years related experience or an equivalent combination of education and experience.
3 years experience as a Manager; or 3 years experience in a senior leadership role.
Strong management skills with the ability to lead cohesive and productive teams.
Strong interpersonal skills with the ability to communicate with all levels of management through diplomacy and tact.
Excellent oral and written communication skills
