Skilled Nursing Facility (SNF) experience preferred
Oakwood Gardens Care Center is seeking a compassionate and resident-centered Social Services Director to support the emotional, social, and care-planning needs of our residents and families. This role plays a key part in helping individuals navigate transitions, access resources, and maintain the highest quality of life in a skilled nursing setting.
Key Responsibilities
- Conduct psychosocial assessments and create individualized care plans.
- Assist residents and families with admissions, ongoing support, and discharge planning.
- Provide counseling, emotional support, and crisis interventions as needed.
- Participate in interdisciplinary team meetings and care conferences.
- Document all social services interventions per federal, state, and facility regulations.
- Coordinate community services, mental health resources, and outside agencies as needed.
- Advocate for resident rights, dignity, and well-being.
