As the Clinical Social Worker , you will provide a range of services to patients / clients and families in inpatient, ambulatory and community settings utilizing individual, family or group modalities and practicing within the guidelines of professional social work ethics and standards.

Responsibilities :

• This position will focus primarily on the mental health and substance abuse population of patients.
• Provides clinical social work intervention on patient care units, inpatient and outpatient, including Emergency Center
• Assesses identified patient’s psychosocial needs and makes recommendations for appropriate treatment plans with issues involving suicide, homicide, family violence, elder / child abuse and substance abuse
• Provides crisis intervention and support for patients / families – identifies community resources and refers appropriately. Assists with discharge planning issues to ensure continuity of care.
• Consults, collaborates and communicates with a wide range of social, governmental and legal agencies, courts, schools, clinics, other hospitals, physicians and other sources. Manages all appropriate referrals to such agencies so as not to impede the plan for timely discharge.
