Responsibilities

Are you ready to make a difference in the lives of individuals with psychiatric needs? As a Social Worker, you'll be the go-to person for providing support and services to our amazing patients!

Responsibilities and Accountabilities :

• Knowledge and Understanding : Have knowledge and understanding of Social Work practices and principles.
• Assessment and Planning : Conduct effective assessments and suicide risk evaluations, develop treatment and discharge plans, and lead educational or therapeutic groups.
• Patient and Family Communication : Maintain open lines of communication with patients and their families to ensure they are actively involved in treatment decisions.
• Care Coordination : Coordinate care with outpatient providers during patient admissions and discharges to ensure continuity of care.
• Team Facilitation : Facilitate weekly treatment team meetings, creating and updating master treatment plans based on team consensus.
• Clinical Contribution : Bring a unique clinical perspective to the interdisciplinary team, drawing on training in family systems and social work theory.
• Effective Communication : Communicate clearly and effectively with clinical team members and Directors to ensure cohesive teamwork.
• Cultural Competence : Be knowledgeable about gender, cultural differences, and age-appropriate care to provide the best support for patients.

Qualifications

Compensation for this role ranges from $24-$34 per hour depending on education and experience.

An individual's pay rate within the posted range is determined by several factors, including skills, experience, education, and market conditions.
