Licensed Social Worker - Outpatient dialysis clinics

St. George, UT based

• Primarily based in St. George with travel 3-4 days / month to Hurricane, UT and Mesquite, NV (paid mileage / drive time)
• Social Worker will see patients mostly on Mon / Wed / Fri and occasionally Tue / Thu. No weekends or holidays!
• Must be licensed in the state of Utah in order to work in this MSW role

In this vital role, you will help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional and financial demands of End Stage Renal Disease.

Requirements :

Meets all state required regulations to practice in the dialysis setting.

Master's degree in Social Work (MSW) required with a specialization in clinical practice

Demonstrated knowledge of government and private insurance programs

Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days
