Atlantic Dialysis Management Services is looking for a Full time Social Worker in our Manhattan Dialysis Center.
Provides social case work and support to the ESRD patient and his / her family, so that they maintain a meaningful lifestyle within the limitation of the illness and treatment.
RESPONSIBILITIES
- Conducts psycho-social assessments on each patient and determines social work treatment plan compatible with a total medical treatment plan.
- Responsible for knowledge of the benefits available to patients under Public Law 92-603, Section 2991 for Medicare, as well as Medicaid, Blue Cross and other insurance coverage.
- Assist patients with interpretation of benefits available to these patients and assists patients in obtaining the benefits to which they are entitled.
- Participates in multi-disciplinary conferences and long-tern, short-term care plan of the patient.
- Utilization of crises intervention techniques in accordance with assessed individual and / or family needs.
- Uses psychiatric consultation in selected situations.
- Refers to appropriate public and private community agencies and assists patients and their families in the use of them : (i.e.) transportation, public assistance, vocational rehabilitation, etc.
- Liaison between the Dialysis Center and affiliated hospitals and other dialysis center and collects pertinent information for patients transfer or transient visit to other hospital or dialysis centers.
