Job description
Key duties include conducting assessments, developing care plans, mediating family communication, managing admissions / discharges, and ensuring compliance with regulations.
- Assessment & Care Planning : Complete comprehensive psychosocial assessments upon admission and develop personalized care plans, including 90-day updates.
- Advocacy : Protect residents' rights, address complaints or reports of abuse / neglect, and act as a liaison between residents, families, and staff.
- Discharge & Care Coordination : Facilitate safe discharge planning, coordinate with community resources (e.g., home health, hospice), and work with the interdisciplinary team.
- Documentation : Maintain accurate, timely, and confidential records of social services, progress notes, and assessments in compliance with state / federal regulations.
- Experience : At least one year of supervised social work experience in a healthcare or long-term care setting.
- Knowledge : Strong understanding of Medicare / Medicaid regulations, residents' rights, and, state / federal, 6 rules.
- Skills : Excellent communication, crisis intervention, conflict resolution, and empathy.
