The Bostonian has a supportive team environment.Requirements for Licensed Social Worker : Must possess a minimum of a B.A in Social Work, LSW or equivalent degree with a current social work license. Masters in Social Work (MSW), and LCSW or LICSW higher preferred.1+ years skilled nursing facility experience preferred.Job Duties of Licensed Social Worker : Compiles social history data.Participates in assessments, development and evolution of residents care plan.Responsible for planning and leading interdisciplinary care planning weekly meetings.Documents timely progress notes according to state and federal regulations.Coordinates the discharge planning for residents.Able to interpret and implement established social service policies, procedures and regulations.Coordinates services of community agencies for continued delivery of care upon discharge.The Bostonian truly appreciates our dedicated staff and looks forward to welcoming a new Licensed Social Worker to join our team!We are proud to be an equal opportunity workplace.
