Licensed Social Worker
WILO NJ is seeking a dedicated and compassionate Licensed Social Worker to join our team. This is an excellent opportunity to make a meaningful impact in the lives of individuals and families in our community.
Responsibilities :
- Continuously assesses residents' needs and strengths to provide appropriate support and establish achievable goals.
- Helps residents identify, respond to, and cope with life stressors.
- Hosts support meetings and collaborates with external vendors to organize group classes as needed.
- Maintains communication records for all residents and service notes.
- Schedules annual check-in meetings for individuals and guardians.
- Sends out client satisfaction surveys and documents results.
- Works as a team with other social workers to maintain a comprehensive understanding of the organization and provide coverage for each other.
- Collaborate with interdisciplinary teams to ensure coordinated and effective care.
- Maintain accurate and confidential client records in compliance with all regulations and ethical standards.
- Advocate for clients' rights and access to resources within the community.
- Participate in case conferences, staff meetings, and professional development activities.
