This position will support the chronic disease management programs.Essential Functions And Responsibilities Of The JobTreats patients and / or families in a caring, respectful manner while respecting confidentiality and privacyCultivates a therapeutic relationship with each patient and / or family memberAttends / coordinates appropriate Patient Care Conferences and / or meetings as necessaryActs in accordance with the mission, values, ethical principles and standards of the social worker role responsibilityPractices within scope of practiceComputer literacy with ability to work within several internal and external on line and Web based systems, reports and analyticsMaintains and complies with all Memorial Health Systems policy and procedures.Interact in a positive and constructive mannerPrioritize and multitaskPlacement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications.
