This LCSW will be instrumental in delivering compassionate and comprehensive care to a vulnerable population, leveraging their expertise in mental health to improve patient outcomes. The role requires a deep understanding of the challenges faced by individuals with SMI and the ability to provide tailored interventions through a telehealth platform.
WHAT IS YOUR SUPERPOWER?
- Conduct virtual initial assessments and ongoing care for seniors with serious mental illness (SMI), in collaboration with psychiatrists and other healthcare professionals.
- Develop comprehensive treatment plans, which may include coordinating psychotherapy or other treatment modalities, and assessing the need for referrals to specialized care.
- Collaborate effectively as part of a multidisciplinary team to address the holistic needs of patients, including their physical health, behavioral health, and social well-being.
- Provide patient care and education tailored to the individual’s age, abilities, educational background, and cultural context, often involving family members in the care process.
- Coordinate care with primary care physicians (PCPs) and other medical professionals to ensure a cohesive approach to patient health.
- Offer timely psychiatric urgent care and be responsive to patients’ immediate needs.
- Document all clinical work comprehensively and promptly in the electronic health record (EHR).
- Adapt to a novel care delivery paradigm and thrive in a fast-paced environment.
