We are currently looking for a medical social worker to help ensure the psychological, social, medical and spiritual needs of the residents are met. The position coordinates with other disciplines to facilitate the provision of needed care and services. Essential Duties and Responsibilities:
- Responsible for the overall management, implementation and coordination of the social services program throughout the continuum of care.
- Assists in the development of care plans and counsels residents and family members.
- Observe and understands the residents’ rights and responsibilities at all times
- Participate in interdisciplinary care team conferences and work with other disciplines in developing and implementing service plans.
- Communicates effectively with other team members, public agencies and other external partners as needed.
- Initiate and update all required assessments per regulatory requirements and Lakewood policy.
Minimum Qualifications:
- Bachelor’s degree in Social Work or other human services field as appropriate
- LCSW preferred.
- Experience in a Skilled Nursing/Assisted Living community with older adults is preferred.
- An understanding of geriatric care and knowledge of the aging process.
- Strong communication skills– oral and written
