Job Summary :
The Medical Social Worker will provide psychosocial assessments and counseling services to hospice patients and their families under the direction of the attending physician. This role involves collaborating with the Interdisciplinary Group / Team (IDG / T) to coordinate care and ensure that patients receive holistic, compassionate support tailored to their needs. You will also participate in an on-call rotation as required by the needs of the hospice.
Key Responsibilities :
Master’s degree in Social Work (MSW) from an accredited school of social work, OR
Current state licensure as a social worker (LCSW, LMSW, or equivalent) required, MO and KS licensure preferred
Minimum two (2) years of experience in a healthcare setting, preferably in hospice or palliative care
Excellent interpersonal and communication skills
