Job Summary :

Definition of Shared Governance : Shared governance is a professional structural model, founded on the cornerstone principles of partnership, equity, accountability, and ownership that form a culturally sensitive and empowering framework, enabling sustainable and accountability-based decisions to support an interdisciplinary design for excellent patient care.

Essential Job Duties :

These statements are intended to describe the essential functions and related requirements of persons assigned to this job. They are not intended as an exhaustive list of all job duties, responsibilities and

  • Demonstrates understanding of hospice philosophy and nursing care principles as related to the admission process.
  • Understands the shared responsibility and concerns of all disciplines as related to the diversity of human experience in hospice.
  • Evaluates incoming patients to the program and determines the intensity of services needed during hospice care.
  • Serves as a positive role model to the program to represent as the first caregiver educating and supporting the patient and family in the plan of care for services to hospice.
  • Able to collect initial information on the medical history, medication history as well as the spiritual, cultural and psychosocial assessment of the patient.
  • Able to articulate to the patient as well as the family the initial information needed on expectations during the hospice process as well as through the death and dying process.
  • Demonstrates timely, accurate, and appropriate documentation of the initial nursing care admission visit as well as documents necessary for compliance with hospice care participation.
  • Communicates effectively with the interdisciplinary team to ensure that the nursing assessment information is conveyed to all disciplines caring for the hospice patients and families.
  • Serves as a clinical resource to nursing home providers concerning hospice care and practices by assisting in the on-going in-services for staff in those facilities regarding hospice care.
  • for nursing council or committee and be vested on one council at least every 5 years. 80% attendance in person or by phone is required. Advancements in Clinical Ladder will be reflected in yearly application process and is expected for job performance standards.

Secondary Job Duties That May be Reassigned :

  • For the purpose of improving care of the terminally ill, develop a hospice presence in the hospital as well as nursing facilities, attending and facilitating rounds and care conferences when possible, conferring with providers, speaking at in-service meetings, attending committee meetings, and participating in new staff orientation.
  • Acts as a liaison and advocate for hospice patients admitted. Solicits feedback on an ongoing basis for both personal and programmatic performance in hospice care.
  • Utilizes feedback for quality improvement in the admission process.
  • Assess each consult to determine the appropriateness for hospice admission, based on individual hospice criteria.
  • Inform patients and their families about the hospice philosophy, goals and services.
  • Seeks opportunities to participate in community programs as related to hospice care.
  • Effectively communicates with internal and external customers.
  • Contributes statistical data for timely and accurate reports to reflect programmatic activities.
  • Identifies professional development needs and seeks opportunities to maintain and enhance knowledge.
  • Recognizes individual role as a representative of the agency and conveys a positive organizational image.
  • Promotes and contributes to a positive work environment.
  • Job Specifications :

  • Education : RN licensure; Bachelors Degree in Nursing within 3 years of employment (with the exception of staff previously granted grandfather status).
  • Licensure : Indiana and Illinois RN license
  • Experience : At least one year clinical practice as a staff nurse prior to employment.
