Qualifications :

Must be enrolled in an active and accredited program at an affiliated school.

Additional Information :

All required immunizations must be submitted (MMR, Hep B, Varicella, COVID (No longer required, but if you have them), Flu vaccination, Negative 2-step TB skin test, and Tetanus).

• Please note your TB skin test, must be a 2-step TB test or a blood draw.
