Job Description
Job Overview :
This position is responsible for providing individual, family and group counseling for Hospice of Cincinnati bereaved family members / caregivers as well as bereaved clients from the community. The Bereavement counselor provides short term supportive grief counseling based upon bereaved's grief issues and concerns. Bereavement counselors facilitate a variety of grief support groups which vary in length of time as well as issue complexity. This position works closely with the community and develops, nurtures and utilizes collegial relationships with community professionals to use as a referral source for clients when grief is no longer a primary issue. Participates in planning, coordination, implementation and management of programming activities necessary to support the mission of the Goldstein Grief Center. Other duties may include follow-up for bereaved who have been designated as high risk and other duties as assigned.
Job Requirements :
Master's Degree in Counseling, Social Work, or Chaplaincy
LPC, LSW, or BCC
Knowledge of supportive counseling principles with children and adults
Knowledge of community resources and training in grief and loss, crisis intervention, stress reduction
Computer aptitude and knowledge of computerized medical records
2-3 years experience Professional Counseling, Social Work, or Chaplaincy
Job Responsibilities :
Assess all bereaved clients presenting for services. Skills reflect appropriate interventions for bereaved designated as high risk and knowledge / identification of complicated grief reactions. Develops care plan and provides ongoing support based upon assessment and client goals.
Provides supportive grief counseling on an individual basis to bereaved family members as well as community bereaved to support grief issues and concerns.
Facilitates grief support groups which may vary in number of sessions as well as type of loss.
Documents all contacts in the electronic medical record; completes bereavement assessments as well as documentation of ongoing contacts. Documentation is timely, thorough and complete and tells the patients story.
Develops collegial relationships to refer bereaved clients to community based professionals when issues outside of grief issues complicate the clients grief work.
Participates in grief support functions including HOC Memorial Services; community presentations are requested including community requests when a community incident has occurred.
Other Job-Related Information :
Must be able to work independently with 2-3 years experience in a counseling setting. Experience in a health care setting preferred.
