Job Description Job Description We are seeking a Therapist Counselor to join our team! You will provide recovery-oriented rehabilitation services in group or individual settings. Responsibilities : Establish open lines of communication for individuals with mental or emotional issues Offer assistance to individuals with substance abuse issues Provide prevention-oriented therapy Implement and review treatment plans Collaborate with additional treatment providers and placement agencies Ensure all documentation is completed in a timely manner DUTIES : Include but are not limited to : 1. Participate in the development and implementation of the consumer's Mental Health Assessment (at the initial meeting) 2. Individual and family session notes every encounter. 3. Individual Treatment Plan (every six (6) months). 4. Provide individual, family, and / or group a minimum of forty-five minutes (45) each session. 5. Attending mandatory bi-weekly meetings with the Clinical Director and a weekly supervision by call or face to face. The First initial month meeting will be required a face-to-face meeting. 6. Coordinate in the creation and implementation of Individual Treatment Plans. 7. Discharge recommendations and summaries. 8. Identify and address the client's therapeutic needs and make necessary referrals. 9. Complete incident report per agency policy and procedures. 10. Provided crisis Intervention when needed. 11. Provides individualized care and treatment that is consistent with / sensitive to the age and lifespan developmental needs of each client. 12. Participate in clinical training, supervision treatment teams and other clinical and educational activities; 13. Participates in treatment team meeting, as needed. 14. Work collaboratively with psychiatrists, psychotherapists, case managers, rehabilitation coordinators and administrative staff. 15. All other duties necessary in the continued operation of Keys. 16. Perform other duties as assigned. Qualifications : Previous experience in mental health counseling or other related fields Familiarity with community mental health resources Excellent written and verbal communications skills Ability to build rapport with clients Strong leadership qualities Company Description We are an Out-Patient facility assisting members with a variety of behavioral and mental health challenges.
