Job description
We are seeking an experienced Nurse Practitioner to join the Risk Adjustment team, specializing in end-stage kidney disease (ESKD) care. As a Risk Adjustment Nurse Practitioner, you will play a crucial role in case management for patients with kidney disease, providing comprehensive assessments, education, and care. Additionally, the NP will collaborate with nephrologists and the care team to complete rounding in the dialysis centers alongside the care team monthly. The main role involves rounding to meet and treat patients and conducting annual wellness visits for the value-based care risk adjustment program KidneyLink, which will occur at the Dialysis Center or Practice Office. The job includes travel to Houston and Rio Grande Valley TX one week a month for the first few quarters. It may include telehealth (20% of the time) and home visits (10-20% of the time) as needed.
