What is the specific title of the position? Pediatric / OB Social worker. Pediatric / OB experience is preferred, but is not required.

Work location- Telecommute in the city of Las Vegas.

Work hours- 8-5 Mon-Fri

Please describe the team the candidate will be working with (number of team members break-down of the team?s skill sets team culture etc.). It is a Pediatric OBGYN team. There is one other social worker and there are 6 RN case managers on the team.

Purpose of Job (Briefly state the primary purpose of the job in terms of how it contributes to the departments objectives) : Responsible for coordinating social services for health plan members. Collaborates with healthcare delivery stakeholders and members throughout the continuum of care. Develops individualized plans that optimize the psychosocial and financial factors impacting care. Acts as a resource for the member and the health care delivery team, identifying and managing referrals to community resources.

Job Components (List the major job responsibilities, accountabilities, and key responsibilities in order of priority. Also include any relevant scope measures as they pertain to the duties listed below) : Major Responsibilities 1. Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services. 2. Manage social service plan of care throughout the continuum of care as a single point of contact, including outpatient services. 3. Document all member assessments and services provided, including referrals to social service programs and follow up documentation to ensure member completion of referral. 4. Meets with member in their home or at their providers' office if requested, deemed appropriate or warranted. 5. Perform other duties as assigned. (e.g., Assist health plan members in inpatient settings, assist the inpatient case management team(s) with resources to facilitate safe transitions of care, participate in Interdisciplinary Team meetings and Utilization Management rounds as necessary, etc.)”

Qualifications (List minimum work experience, educational background, professional licenses, certifications, and skills required to perform the job) :

  • Bachelor's Degree in Social Work required.
  • State of Nevada Social Work license required.
  • Minimum two or more years of related social work experience in an acute care setting, clinic, home health or case management setting.
  • Must demonstrate a working knowledge of social work theory and practice and how to use a variety of diagnostic tools and techniques.
  • Must have a basic knowledge of both state and federal programs and the community-based service continuum and the rules and regulations applicable to these programs and services.
  • Knowledge of advanced directives and legal regulations, including reporting of abuse, required. BEH147 HPN / Continuity of Care 25
  • Understanding of managed care preferred.

Competencies and Best Practice for High Performers (List competencies and at least two behavioral anchors for each) :

  • Skills in building and maintaining community resource database by establishing effective liaisons with key sources of assistance for members and families required.
  • Intermediate computer skills including experience with at least Microsoft Word and ability to navigate in a Windows environment.
  • Ability to communicate appropriately with diverse member populations and to be flexible and supportive to member and family needs.
  • Ability to work in varied settings with minimal supervision required.
