Job Title : Physical Therapist (Part-Time)

Position Summary
We are seeking a Physical Therapist to provide exceptional care to clients in their homes. You will assess, plan, and implement physical therapy services in accordance with the Plan of Care and agency policies. In some cases, you may also serve as the case manager for clients requiring skilled therapy services only.
Key Responsibilities
Perform physical therapy assessments, diagnostic tests, and skilled procedures for clients under a medically approved Plan of Care.
Develop and update therapy care plans, ensuring they align with client needs and agency policies.
Educate clients and caregivers on treatment plans and resources to promote recovery and symptom management.
Document all findings, interventions, and outcomes, submitting reports within agency timelines.
Communicate with physicians, case managers, and interdisciplinary team members regarding care plans and changes.
Supervise and provide on-site guidance for Physical Therapy Assistants and Home Health Aides.
Promote safety and infection control practices in compliance with agency policies.
Qualifications
Graduate of a Physical Therapy program accredited by the American Physical Therapy Association (APTA).
Licensed as a Physical Therapist in the state(s) of practice.
Minimum of 3 years of healthcare experience (home care experience preferred).
Current CPR certification.
Strong verbal, written, and interpersonal communication skills.
Ability to work independently with minimal supervision.
Licensed driver with an insured vehicle and a good driving record.
Physical / Environmental Requirements
Ability to sit, stand, bend, and lift up to 50 pounds.
Frequent use of telephones, computers, and clinical equipment.
Must travel to client locations and attend occasional off-site business meetings.
