This is a union position.
The Specialized Treatment for Optimized Programming (STOP) Program Area Six connects California Department of Corrections and Rehabilitation inmates and parolees to comprehensive, evidence-based programming and services during their transition into the community, with priority given to those participants who are within their first year of release and who have been assessed to as a moderate to high risk to reoffend. Area Six includes San Diego, Orange, and Imperial counties. STOP subcontracts with detoxification, licensed residential treatment programs, outpatient programs, professional services, and reeentry and recovery housing throughout the program area to assist participants with reentry and recovery resources.
The Transportation Coordinator will oversee and coordinate transportation services with CDCR referrals from in-prison programs, SATs, Reentry Hubs, Reentry and Revocation Courts, and other locations as requested by CDCR. Schedule relay services with other STOP Program Areas, develop Transportation Plans, oversee vehicle maintenance, and enter transport information into database. Coordinates transportation services and / or drives participants from CDCR institutions, County Jails, Parole Offices, local depots, and other locations such as assisting current participants with transportation to scheduled appointments pertaining to their reentry efforts as requested by the STOP 6 Placement Coordinator. Provides support in all areas related to vehicle records and maintenance.
Key Responsibilities
Transportation Responsibilities : Coordinates transportation of participant. As needed, safely transport participants in company provided vehicle following all applicable federal and state driving laws. Always maintains appropriate staff / participant boundaries.
Vehicle Maintenance Responsibilities : Responsible for ensuring routine vehicle maintenance including inspections, interior and exterior cleaning, and pumping gas. Maintains all such documentation. Enters transportation activities in company database, files related paperwork, maintains, and prepares vehicle usage reports and logs. Maintains and reconciles invoicing and backup documentation for transportation services. Maintains accurate vehicle documentation with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.
Transportation Coordination Responsibilities : Maintain transportation board, assign runs to STOP drivers, review board to insure drivers are confirming runs with both releasing and receiving entities, coordinate pick-ups with transitional staff from releasing institutions (SAT, County, CBP’s). Review client profiles to identify potential difficulties impeding transportation (medical, disability, psychiatric) and assign appropriate driver / vehicle. Communicate issues affecting the ability of transporting clients to immediate supervisor. Must be available for possible evening and weekend shifts.
Database Management Responsibilities : Creates, opens, updates, closes and archives client files in accordance with department procedures. Assist in establishing and maintaining an effective and efficient records management systems. Ensures participant files meet contractual compliance Maintain and comply with confidentiality requirements. Oversees data entry into various electronic systems in accordance with guidelines established by HealthRIGHT360 to satisfy internal and external evaluating requirements. Conducts file compliance and reconciliation.
Contacts preparers of source documents to resolve questions, inconsistencies, or missing data. Confirms accuracy of data such as course numbers, days, dates, times, locations, and instructors. Compares data entered with source documents, or reenters data in verification format on screen to detect errors. Reviews and make necessary corrections to information entered. Compiles, sorts, and verifies accuracy of data to be entered. Maintains confidential information.
Administrative Responsibilities : Keeps track of received data and source documents. Prepares and sort source documents and identify and interpret data to be entered. Performs general clerical duties such as typing, answering phones, filing etc. Process various forms. Generate reports and respond to inquiries regarding entered data as requested. Attends required training and meetings. Communicates with California Department of Corrections and Rehabilitation (CDCR) personnel. Maintains project documents and records. Submit s reports to Program Manager. Conducts ongoing communication with a large network of subcontractors serving the population.
Placement Responsibilities : Assess, schedules and coordinates placements of project referrals into appropriate community placements. Coordinates transportation with Dispatcher. Matches inmates to the appropriate level of care within a network of sub-contracted community-based providers. Screens paperwork for possible obstacles to treatment provision. Receives transitional plans from CDCR and County institutions. Conduct daily communication with Parole Agents. Conducts presentations to community agencies to increase the pool of referrals. Identifies community capacity needs. Revises and submits placement requests.
Incident Reporting Responsibilities : Ensures the accuracy, confidentiality and timeliness of all incident reports submitted. Contact CBPs to request timely and sufficient reports. Reports urgent incidents to supervisor and revises and submits individual incident reports to DRP daily, resubmits reports with updates as needed. Contacts AOR for
