Summary
We are looking for experienced, dedicated RNs to join our home health and / or hospice team. you will play a vital role in delivering exceptional patient care. This position is essential for ensuring that our clients receive the highest standard of medical support while promoting their independence and well-being.
Responsibilities
- Provide comprehensive patient assessments and develop individualized care plans.
- Administer medications and treatments as prescribed by healthcare providers.
- Monitor vital signs and report any changes in patient conditions promptly.
- Educate patients and families on health management and wellness strategies.
- Collaborate with interdisciplinary teams to ensure coordinated patient care.
- Maintain accurate documentation of patient progress and interventions.
- Participate in utilization management activities to optimize resource use.
- Support pediatric, outpatient, and primary care initiatives as needed.
