Summary

We are looking for experienced, dedicated RNs to join our home health and / or hospice team. you will play a vital role in delivering exceptional patient care. This position is essential for ensuring that our clients receive the highest standard of medical support while promoting their independence and well-being.

Responsibilities

Provide comprehensive patient assessments and develop individualized care plans.
Administer medications and treatments as prescribed by healthcare providers.
Monitor vital signs and report any changes in patient conditions promptly.
Educate patients and families on health management and wellness strategies.
Collaborate with interdisciplinary teams to ensure coordinated patient care.
Maintain accurate documentation of patient progress and interventions.
Participate in utilization management activities to optimize resource use.
Support pediatric, outpatient, and primary care initiatives as needed.

Qualifications

Valid nursing or healthcare professional license appropriate for the role.

Experience in hospice or home health is preferred.

Strong understanding of medical terminology and vital sign assessment.

Ability to manage multiple tasks efficiently in a hospice environment.

Excellent communication skills for effective interaction with patients and families.
